Solid choice in most of the games. Helps against assassins, divers and melee overall. It also lowers your healing output as you do not take Heal. Solid choice overall.
Ignite
Offensive option, improves your damage, gives you additional cooldown to use in fights, but reduces your healing output.
Ghost
Improves mobility. You can chase, position and flee better. Reduces your healing output, but makes you more agile in fights.
You play aggressively, getting Echoes of Helia stacks and spending them as soon as you can.
You can proc this item consistently with Guardian and Font of Life rune, Heal summoner spell, items and abilities.
You adapt to your team's needs: more support, more damage or even more tank.

How to play
Helia Mordekaiser
Enemies won't expect Mordekaiser support. They will assume you're playing standard, so you'll draw focus in fights - use that to bait cooldowns or redirect pressure from your carry.
Utility
Your Death's Grasp (E) can be used to pull enemies out of position, cancel dashes, or disrupt channeling abilities. This makes it both a peel tool and an engage tool.
Damage
Your Darkness Rise (Passive) deals max health damage, which makes you extremely strong against bruisers and tanks. Even without any damage items you are gonna hurt them.
Realm of Death (R)
Your Realm of Death (R) is your most powerful tool in ARAM. Use it to remove a dangerous enemy from the fight: take away their ADC, AoE ult, their assassin, or even a tank about to frontline.
You can also use Realm of Death (R) defensively: if an assassin dives your backline, trap them in the Death Realm. Even if you die, you’ve prevented them from deleting your carry.
Smart Realm of Death (R) use wins fights. Sometimes it’s better to hold an enemy out of the fight instead of killing them. A 4v4 without their strongest champ is a free advantage.
This is an off-meta and very fun build that works on ANY champion - even those without healing or shielding abilities!

Items
Get Echoes of Helia #1. Then you need to adapt in the game to your team and to the enemy team.
Usual item order: Echoes of Helia -> Moonstone Renewer -> Redemption -> Locket of the Iron Solari then adapt.
Counter enemy team, go more support or go more damage. You can even build some tank items if needed.
Buffing your team
Ardent Censer is worth it if you have auto attackers on your team (ADCs, auto-attack bruisers). It will trigger often enough to be good buy.
Bandlepipes is a good new item that gives MS and buffs your allies Attack Speed when you slow/immobilize enemies. Good for your auto attackers.
Staff of Flowing Water is great if you have AP champions on your team. You'll be proccing it frequently with your support abilities.
Adaptation
Your team needs to engage or position? Get items like Shurelya's Battlesong or Trailblazer.
Your team getting poked to death? Consider getting Redemption AND/OR Locket of the Iron Solari next.
If you are just getting poked and cannot manage to stay alive - get Kaenic Rookern or Warmog's Armor (caution: needs enough HP!)
You have tanks, bruisers and your team lack damage? As a last resort, you can buy few items to even help with that!
Remember to counter an enemy team. For example, if they have lots of healing, get Oblivion Orb early.
Tips and Tricks
Champion
You can be pretty good at peeling from your Team or Carries. Enemy has Fiddlesticks? Maybe Zed that jumps on your Ashe? Just ult them with your Realm of Death (R).
Riot
Offense is your defense. Keep fighting to build up larger Indestructible shields.
Hitting multiple champions with the same ability can help quickly activate Darkness Rise.
Use Realm of Death on a low health enemy to guarantee the kill and keep their stats for the rest of a teamfight.
